2018 State Health Benefit Plan Rates
Be mindful that the rates do not reflect the $80.00 tobacco surcharge.
State Merit will not be printing any booklets or material this year. You are encouraged to take time to read the material and give some thought as to which plan will best suit your needs. Blue Cross Blue Shield, Kaiser and United HealthCare will each have plans available.
If you take No Action, you will remain in the same Plan Option, same tier and remain with the same vendor. If you are paying a surcharge it will continue, and if you are not paying a surcharge you will continue to not pay.
In reviewing the Guide and Rate page, you will notice that little has changed. There is an increase in rates.
Due to limited changes, only two meetings will be held:
• Wednesday, October 18th 3:45 p.m. High School Auditorium
• Thursday, October 26th 8:00 a.m. Bus Shop
Mike Martz will be visiting each facility to meet with staff. Whether making changes, or not, Mike will need your signature on the election page. Mike handles Unum Life Insurance, Hartford Disability, Superior Vision, Delta Dental, Allstate Cancer and Medical Reimbursement.
Scott Hayes, AFLAC Representative, will also visit the schools to provide information on AFLAC policies.
Melissa George, Colonial Representative, will also visit the schools to provide information on Colonial Life policies.
